BOOK AN APPOINTMENT

Cancellation of an Appointment: Clients must provide Exten "Clinic" with no less than 12 hours prior notice in the event you need to cancel an appointment. If you provide no notice or less notice than required, the Clinic is entitled to charge account a missed appointment fee of up to $100. The Clinic will bill card on file for such cancellation charges, and if no card is on file, we ask you pay the cancellation fees on the earlier to occur of next appointment at Clinic or within 20 days after receipt of invoice from Clinic.
